Our Care Services

ACS Home Care LLC provides unparalleled professional home care and elder care services in the greater New York area.

We help those in need to remain independent in their own home, when possible, and also provide care at nursing homes, hospitals and other care facilities. We offer flexible scheduling to accommodate each individual's unique situation.

ACS' caregivers can be retained on a live-in or hourly basis. Perhaps you need just a few hours a week to assist with bathing and other routine activities or maybe you require 24 hour a day care/7 days per week? Whatever the situation is, ACS can assist in providing the right caregiver.

Alzheimer's Care Services

We assign a Nurse Case Manager to meet with each new client suffering from cognitive impairment to perform an assessment of their specific needs before our professional caregiver becomes involved.

Once we have a clear understanding of what stage of the illness the client is at, we then initiate a personal care plan for our trained caregiver to follow. This personal care plan, approved by the patient's physician, serves as a guide so that we may supervise the care we are providing and ensure that we are meeting the client’s changing needs.

This personal care plan includes an activities program for clients with cognitive impairment. Depending on the stage of the disease, these activities cover such areas as creative art, daily living skills, physical activities and sensory activities. Each activity in the program is flexible, allowing each client to experience the best possible success and enjoyment at their capability level.

Our professional caregivers are highly skilled in caring for clients with Alzheimer's disease and other dementias. Having successfully completed the Dementia Care Training Program created by the Alzheimer's Foundation of America, they have all earned the designation of "Qualified Dementia Care Specialist." In addition, all ACS Aides are insured, bonded and undergo an extensive background check.

Additional Home Care Services

Home Care Aide 1 - Homemaker / Companions

Typically, people recovering from a minor illness, or those that require minimal assistance to remain safely at home, may receive these services. Please refer to the list on the right for information on some of our basic companion services.

Home Care Aide 2 - Personal Care Attendant / Nurses Aide

People who have had strokes, heart problems, serious surgeries or are recovering from a critical illness require the skills provided by our Personal Care Attendant/Nurses Aide. In addition to performing the duties of a Homemaker/Companion, this professional performs the following:

• Preparing a special diet & assisting with feeding
• Assisting with bathing, dressing, hair care and grooming
• Monitoring and reporting skin condition
• Monitoring food and fluid intake
• Assisting with toileting and monitoring elimination
• Assisting with ambulation and transfers

Home Care Aide 3 - Certified Nurses Aide / Certified Home Health Aide

These services are for clients who choose to be at home for intensive rehabilitation or are very ill. Our professionals are qualified to carry out the duties of a Homemaker/Companion, Personal Care Attendant/Nurses Aide as well as:

• Assist with exercises and rehabilitation activities
• Empty and care for the catheter or colostomy bag
• Assist with special assistive devices (i.e. canes, walkers, wheelchairs, artificial limbs)
• Assist with simple procedures as directed by the plan of care
• Identify and report signs and symptoms needing attention.
